The purpose of this autobiographical sketch is to allow the reader/reviewer of this portfolio to
become more acquainted with you in a short time frame. Keep the focus of your sketch on what
you want the reader/reviewer to know about you as a future high school graduate. You are
answering the question of why I want to pursue a career in Heath Profession                (you
complete the blank). Is there anything in your family background or in your life experiences that
will help you answer that question?

Write your autobiography in first person. Write it like a story, not like a resume. You may include

Family background:

Hello my name is Lisa Le and I have a family of six including for course. There are my mother and

father, then my two brothers

and one sister. However, this excluded all my aunts and uncle plus grandma.

Activities:

All the activities that I’m present in are: volunteer work, church related, and after school

Organizations and clubs:

As an after school program I attend in ASA (Asian students Association) (Asian Club) and that may

have been the only one.

Honors:

Out of all my education experience, most to all the class I signed for were honors classes.

Interests and hobbies:

I am interested in read mystery, sometime romance novels. I also enjoy writing my own

drama ( storyline), poems, and sometime songs. In addition, I also like to sing and act (however

only
to myself.) Lastly, some of my hobbies are playing tennis or basketball, working out

Math problem, and helping others.

Church related activities:

The Church related activities I attend are: help at holiday events and church school.

      Hi, my name is Lisa Le the position I would like to apply for is to be an RN nurse at Kaiser.
The reason why you should hire me is because I'm fully train with background experience, also
several practices of what's needed to be performed. In addition I am very responsible and serious
about anything relating to my performance. Lastly the reason why I have chose Kaiser to apply to
is because the nurse at Kaiser are extremely nice and I would like to be apart of that.
      The purpose of this letter is to inform you of my request of applying for a RN position. Before I
requested for an interview, I was informed that there are several of RN position opened, and I’m
the person for it. It is true that all applier are truly up for the position; however are they all really
responsible and serious of what they are applying for as I am. The best example to support my
claim is out of all the years of my education not one time I missed a day, unless it was an extreme
emergency.
      If I was hire be sure of this that you will almost never have to be worried about me for I am
always there on task. Always there doing what I need to do. In addition I am a fast but careful
worker, so in any given condition I am ready to fill in. Also, I am very flexible for any changes in the

I will find the time to perform 1-2 hours of work daily for this course by

         planning events organization and what not. Also office paper working skill.

         2. On holiday events I would volunteer at my community center. This developed the skill of world
         interaction as well as child care.

 G. The Wrap Up (15 points)
 This is where you need to reflect on all the information you have gathered and write a paragraph(s) that tells
 me why you are or are not still interested in pursing the career you have researched. It should be typed and
 approximately 200-300 words.
             After the research I felt more confident that I would like to approach my career as becoming a
 nurse. First, I would like to state that becoming a RN has always been my interest. In addition, there was no
 other career that created this interest. From the working condition (working environment) to the salary all
 have fit my interested. Second, I’ve always wanted to be able to help other that was science related for
 there was no other subject that I like best. Being able to become a nurse would have combined my two
 interests, which are helping people and science related. Even thought, I have been interested in this career
 for long amount of time, but without this research I would have not been able to understand more about the
 career I would like to attend to. Item such as taking courses like: Anatomy, Physiology, Microbiology,
 Chemistry, Nutrition, and Psychology. Third, this research also had a positive effect on my education in the
 future; I am now to obtain a larger list of choices for my education. Finally, all of the reasons above support
 my decision to become a nurse. In addition the research created a more confident feeling towards my
 choice rather than just the support from others. I am now more certain that I will be able to do so.

        -My freshman year, as for how it seems as if that something I have always wanted to do.

        -The most rewarding is know that you helped someone and the most challenging may just
        be the patient attitude.

          - I do: first I will enter the position of a nurse then work and learn at the

and spelling.

1. Describe the assignment including the tools and process.
        This assignment was very helpful for future experience. Now I have a basic idea of how
much work it is to complete something similar to this. In addition the practice of writing atcover
letter was really helpful as well as the resume and autobiographical sketch. These all were helpful
because it created an opportunity for me to better understand myself and a determination of what I
want to do in the future. The main part that helps me be more determined about my career is the
career research assignment.

2. Write about what you have learned from this assignment. Describe the skills and competencies
demonstrated by this assignment.
          Even thought it is very sad that I just recently learned that there are different type of being
a nurse, however that what this project help me understand more about my career as LPN or RN.
In addition this project created the opportunity of an easily search for a more certain decision.
Another thing that I have learned from this project in greater detail of how to write a cover letter, not
many course I taken explain of this. Now with the new skill I have learned can better of what I have
already know. Lastly, this project really help me learn more about myself, especially, when I had set
my goals for this classes help me learn of what I want for my future.

3. Evaluate your performance on the assignment. Identify what you did well and what could be
improved.
             I believe that iI did well on the personal statement, cover letter, and the resume for
those are item that I have done before. Overall all the assignment in this project were straight
forward. However, I could have improved in how I preformed my research performance. Also, the
willingness of completing this project. Lastly, I could have improved the quality of this project.

Personal Statement/Autobiographical Sketch By Lisa Le The purpose of this autobiographical sketch is to allow the reader/reviewer of this portfolio to become more acquainted with you in a short time frame. Keep the focus of your sketch on what you want the reader/reviewer to know about you as a future high school graduate. You are answering the question of why I want to pursue a career in Heath Profession (you complete the blank). You may include such things as: Family background: Hello my name is Lisa Le and I have a family of six including for course. There are my mother and father, then my two brothers and one sister. First, I would like to state that becoming a RN has always been my interest. In addition, there was no other career that created this interest. Being able to become a nurse would have combined my two interests, which are helping people and science related.
